1. Age Verification & Model Release Form 
Go to the Age Verification Form Below. Print out the form, fill it in and bring it with you to the filming or photo shoot, along with a photo copy of your drivers license or valid state ID. We want to be absolutely sure that you are in compliance with the adult laws in the USA (where our company resides), this is for your protection as well as ours. This form is required by law for all persons that will appear in video and/or picture shoots used on our website.
